Select the fifth row “Nikaido Basketball School”.
Enter the participant’s name ‘Surname’ then 'First Name’.
Write your mail address.
Provide your local phone number. (If you do not have one, you can give Nanako’s work mobile 070-3057-7750.)
Select your kid’s grade/level.
Press ‘I agree’ button.
